Had a look into this matter for you and for us to help you on this we will need some details from you. Please find these bellow: 

- Battle.net Account Name: The current email address you are using to login. 
- Name: Your full name, that the account was created with. 
- Address: The full address given when the account was created. 
- Secret Question and Answer (Q:" What was the first car you owned?") 
- Last 4 digits on the visa/credit/debit or pre-paid gamecard that you used 
- A scanned copy of a valid photo ID (passport, national ID) **needs to have the expiry date and your picture on it, please do not crop the pictures. 

Please attach these details to your reply in a .txt file(for data related info) and a .jpeg file(for picture related info)
